To ensure that everything runs smoothly on the day of your wedding, I require all my clients to fill out the following form.

There will be a total of 3 PARTS which will require information about your wedding day, shot requests and selected vendors. Please double-check to make sure everything is correct and all required fields are filled out before submitting the form.

After submission, a copy of your form will be automatically CC’ed to your email for your records. Should there be any changes after your submission, please inform me via email ASAP. Thank you!

    Wedding Coordinator / Emergency Contact

    If you did not hire an event coordinator, please choose a person who will act as General Manager. This person should NOT be your Venue coordinator, because they cannot manage or make decisions related to your timeline. A Venue coordinator is employed by the venue, not by you, and they are mainly concerned with the venue's operations. Assume that you will both be unreachable and enter the next best person for me to contact in case of emergencies.

    The Standard Shot List

    If there are any shots that are NOT on this list that you'd like me to capture, please list them in the appropriate box below. This will help me prepare and keep an eye out for person(s)/things that are important to you. I cannot guarantee that I will get everything, but I'll do my best!

    Group Shot Arrangements

    These are the shots that are usually taken right after your ceremony with your family and guests. Please indicate the approximate number of people in each group in a bracket. eg: Bride & Groom + Work Colleagues (10). Arrange the list so I shoot the larger groups first, that way we can clear the ceremony area faster. I recommend this order: Friends/Colleagues > Extended Family > Immediate Family > Parents
     

    Sensitive Situations

    Are there any situations that I should be aware of when I'm arranging people? (eg. deaths in the family, divorces, family feuds, individuals with mobility or visual impairment etc.)
     

    Group Shot Assistant

    To maximize efficiency, please ensure that this individual has the Group Shot list on the day of your wedding so they are able to assist me in identifying people and calling out each group.

    PART III : VENDORS & ATTIRE

     

    As a photographer, I try my best to capture all the little details of your wedding day. Whenever possible, I would like to give your vendors due credit for their work, and possibly provide them with photos for their portfolio. I would appreciate it if you could fill out as much information as possible about your selected vendors, so I can credit and refer them wherever necessary.
     
    If your vendor is a family member or friend who would like to be contacted for future opportunities, please include a phone number, email or instagram handle so I can refer them accordingly.

    At least 2 weeks prior to your wedding date, please remember to send the following documents to me via email:

    • Digital copies of all portrait location permits

    • Wedding Day Timeline (please highlight when you would like me to show up)

    • Any rules/guidelines for the Ceremony/Reception Venue (if applicable)

    • Reception seating floor plan with important individuals and vendor table highlighted (if applicable)
